2) Ensure to include your Pharmacy’s name in the free text at the bottom
3) Your Prescription will be sent electronically to your Pharmacy within 3 working days

Private prescriptions are subject to fees. All fees are payable at time of collection. If you would like your prescription faxed or posted please call us and provide payment details over the phone.

GMS patients and patients with Doctor Visit Cards will not be charged for prescriptions

Please note the resulting prescription from the request form sent to your doctor may vary from the request made by you, the patient. You, the patient, remain under the care of your GP who will monitor and advice on all medications prescribed by them to you. The GP has the right to alter repeat prescriptions in the interests of their patients.

Only prescriptions that have been written by Slievemore GP will be repeated. Patients on repeat medications will need to be seen by their doctor at least once every 12 months. In some cases, depending on the type of medication, you may need to be seen more frequently.
